Baseball house manager evaluation

This evaluation will be used to provide feedback from parents about the HOUSE BASEBALL team managers at Lou Gehrig.  All data will be confidential,  therefore we do not request that you identify yourself.

Please complete this form and click the "Submit" button at the bottom of this page. All surveys should to be completed by September 15th of this year.  This information is used to better help us choose managers for the following season.   If your evaluation is not submitted, we will assume that you "strongly agree" with all the statements.
