This evaluation will be used to provide feedback from parents about the TOURNAMENT BASEBALL team managers at Lou Gehrig.  All data will be confidential,  therefore we do not request that you identify yourself.

Please complete the survey ASAP as we are finalizing Tournament Managers in the next few days.  This information is used to better help us choose managers for the following season.   If your evaluation is not submitted, we will assume that you "strongly agree" with all the statements.
