Tournament Baseball Manager evaluation page

This evaluation will be used to provide feedback from parents about the TOURNAMENT BASEBALL team managers at Lou Gehrig.  All data will be confidential,  therefore we do not request that you identify yourself.

All surveys need to be completed by August 15th.  This information is used to better help us choose managers for the following season.   If your evaluation is not submitted, we will assume that you "strongly agree" with all the statements.
